Child death and case reviews

When a child dies unexpectedly several investigative processes are likely to be undertaken. Bolton Safeguarding Children Board is responsible for managing two of these:

  • Child Death Reviews – review all deaths of children and young people under 18
  • Serious Case Reviews – review the cases where children have died and abuse or neglect are known or suspected to be a factor in the death or a child has been seriously harmed and abuse or neglect is known or suspected to be a factor and there is cause for concern as to the way in which the authority, their Board partners or other relevant persons have worked together to safeguard the child

Both processes are used to:

  • Understand fully the reasons for the child’s death
  • Address the needs of other children in the household
  • Address the needs of all family members
  • Consider lessons to be learned to inform future planning to safeguard all children in the area

Any agency or individual can discuss a child death with Bolton Safeguarding Children Board to consider whether a review should be considered. For further informaiton or a discussion you should refer to the resources or contact:
