Integrated Working & Early Intervention
Early intervention and integrated working are an essential part of supporting and safeguarding children and young people within their families. Working with families at an early stage to help identify and assess individual needs means that support can be delivered more quickly to prevent things from getting worse. Standard ways of working have been developed to ensure consistent practice across services. These include:
- Information Sharing Practice – ensuring that information is shared to support good practice and that this is done legally and professionally to achieve improved outcomes
- Common Assessment Framework – a shared assessment and planning framework for use across all children’s services promoting co-ordinated service provision to meet needs
- Lead Professional Role – the appointment of a key practitioner who is already involved with a child or their family to make sure they are kept informed and involved. The Lead Professional also co-ordinates the delivery of actions following assessment to achieve intended outcomes.
The Integrated Working Team supports the establishment and embedding of Integrated Working Processes by providing advice and support, guidance documents and training.
