Serious case reviews

Serious Case Reviews are always carried out by Bolton Safeguarding Children Board (BSCB) where a child has died and abuse or neglect are suspected to be a factor in the death.

BSCB can also conduct a Serious Case Review where one or more of the following criteria are:

A child sustains a potentially life threatening injury or serious and permanent damage to their health and development through abuse or neglect
A child has been subjected to a serious sexual abuse
A parent has been murdered and a homicide review is being initiated under the Domestic Violence Act 2004
A child has been seriously harmed following a violent assault perpetrated by another child or adult

And the case raises concerns about how organisations have worked together to safeguard and promote the welfare of the child.

Serious Case Reviews have been developed to try and learn lessons from these tragedies improve services and potentially prevent future tragedies.

Any agency or individual can discuss a child death with BSCB to consider whether it meets the criteria for an Initial Serious Case Review Panel. You should contact:
